Internationalization of Psychology Questionnaire
CONCENT FORM Please read the following consent agreement. I agree to participate in the research study entitled "Internationalization of Psychology: Challenges and Possibilities" that is being conducted by Dr. Alan E. Stewart and Ms. Azadeh Fatemi (Department of Counseling & Human Development at the University of Georgia, Athens, GA, USA, phone: 1-706-542-1812). I understand that my participation is entirely voluntary. I understand that I can refuse to participate or withdraw my consent at any time without penalty or loss of benefit to which I am otherwise entitled, and decline to submit my responses to the online questionnaire that follows. I indicate that my participation is voluntarily and will fill out the survey in English. I understand the following points: 1. PURPOSE: In this project we plan to identify different challenges as well as possibilities that psychologists around the world experience in the process of internationalization of psychology. Knowing these problems, challenges and possibilities are very important since this knowledge is the first step in planning for resolving the challenges in the future. Also, understanding these issues can help psychologists in different places in the dissemination of their research findings, which can increase the depth and richness of psychology as a field. 2. BENEFIT: A benefit of participating is the possibility of thinking and reflecting on the professional challenges that I as a psychologist have experienced. Furthermore, at the conclusion of my participation, I can ask for an explanation of the topics that were investigated in this study. As a larger benefit to the societies around the world, this research may help increase the general awareness of the problems in the process of internationalization of psychology that may lead to addressing or solving these problems in the future. 3. PROCEDURES: I will be asked to respond to a questionnaire. This can be found in the email attachment as well as online through the world wide web. This will take approximately 10-15 minutes to complete, depending upon the individual. The survey will begin by gathering some demographic information. Beyond this introductory information, the questionnaire will inquire about my understanding of the challenges and possibilities in the process of internationalization of psychology. 4. DISCOMFORTS or STRESSES: No discomforts or stresses are associated with completing this questionnaire. 5. RISKS: No risks are foreseen. 6. CONFIDENTIALITY: Once I complete the questionnaire and submit my responses, only the researchers will have access to the database. Potential participants must remember, however, that Internet communications are insecure and there is a limit to the confidentiality that can be guaranteed due to the technology itself. After we receive an email, we will save the attached survey in a folder designed for this study and delete the original email. Also, survey is not linked to personal information. By clicking on the "Agree" button below, I do consent to participate in this study. You may discontinue your participation at any time simply by closing the window and shutting off your computer. Your identity (email information) will not be recorded and the database will be removed from the server every 1-2 days. 7. FURTHER QUESTIONS: Following the completion of the data collection, I can ask for a complete explanation of the study and will be able to contact the researcher Dr. Stewart (aeswx@uga.edu) or Ms. Fatemi (azadeh@uga.edu) if I have any questions. Additional questions or problems regarding your rights as a research participant should be addressed to The Chairperson, Institutional Review Board, University of Georgia, 629 Boyd Graduate Studies Research Center, Athens, Georgia 30602, USA; Telephone 1-706-542-3199; E-Mail Address: IRB@uga.edu.
